To better serve you please review the following before your first visit.
- Please bring your current Phone ID & Insurance Card
- Please arrive 10-15 min before your first visit as some insurance companies require additional forms in addition to our office new patient intake forms.
- Please wear comfortable clothing; we are interested in how you move and understanding what activities cause your complaint. Our exam and treatments are active.
- Please alert us at least 24 hours in advance if you need to reschedule your appointment.
